Included in your kitbox is a Lab Sample Registration form. Please fill this out in full and return it to the ziplock bag and include it in your sample box.
This Registration Form is included with your kitbox. Make sure you fill it out and put it in the ziploc bag (also included). We need this to match your sample with your account when it arrives at the Lab. Without it and we cannot tell your sample from another. Make sure to put registration sheet into the ziploc bag so it doesn't get destroyed by the microbes in the soil while it is in transportation.

Collect your sample material and deposit it directly into the collection box. Include your Sample Registration (place in included Ziplock bag and seal) in your Sample Collection Box. Be sure to collect your sample according to our Sample Collections protocols outlined here.
